Request an ODA Wellness Trust Quote for a New Office

The ODA Wellness Trust open enrollment period is from 10/15/18 through 11/15/18 with an effective date of 1/1/19.  To request a quote during open enrollment:

1. Complete and sign the Health Plan Participation Request for your office by clicking here
2. All employees who would like to request a quote must complete a Personal Health Questionnaire by clicking here
3. Eligible employees not requesting a quote must complete and sign the waiver table.
4. Submit all forms (a Health Plan Participation Request for your office, Personal Health Questionnaires for all employees who would like to request a quote, and a completed waiver table for all employees who are not requesting a quote) via fax to (614) 340-9444. Please include an email address where the fax receipt confirmation should be sent. An email will be generated to confirm receipt of all faxes received within one business day. If you do not receive a confirmation of fax receipt (please check your junk/spam file), we have not received your fax. You may also submit when using a secure email system to insurance@oda.org.   

When requesting a quote, be sure you’re not forgetting any forms: a Health Plan Participation Request for your office and Personal Health Questionnaires for all employees who would like to request a quote. Eligible employees not requesting a quote must complete and sign the waiver table.
